Tourist Visa Application Details:

1. Original passport with at least 6 months validity from the date of intended travel along with the original previous passport (if any)
2. Visa application form has to filled online and appointment for submission has to be scheduled through the following link http://visa.mfa.gov.ua
3. Four latest passport size white background photographs with face close up (do not staple the photograph)
4. Covering letter on the company’s letter head stating name, passport number, designation, the purpose of travel and the mobile number from the applicant
5. Original hotel confirmation voucher along with the payment confirmation receipt
6. Registration certificate copy of the hotel / tour operator in Ukraine
7. Original overseas travel insurance with the sign and stamp pf the insurance company
8. Confirmed return air ticket
9. Original latest personal bank statement dually signed and stamped by the bank of last 6 months
10. Personal Income tax return of the applicant for the last 3 years
11. Relationship proof with the company (i.e. MOA if Director, Partnership deed if Partner, Proprietorship certificate if Proprietor and salary slips of last 3 months in employed)

Please note :

1. Applicants personal appearence is mandatory
2. Prior appointment is required for the submission of the application
3. The Visa fee needs to be deposited to the Embassy’s account in Ratnakar Bank Limited before the applications submission
