1. Copy of the passports first, last page and observation page (if any)
2. 1 passport size white background photographs with face close up
3. Overseas Travel Insurance with a minimum coverage of 30,000 euro
4. Return air ticket itinerary and Hotel reservation voucher

Tourist/Business Visa Application Details:
- Original passport with at least 6 months validity from the date of intended travel along with the original previous passport (if any)
- Visa application form has to filled online and appointment for submission has to be scheduled through the following link http://visa.mfa.gov.ua
- 2 latest passport size white background photographs with face close up (do not staple the photograph)
- Covering letter on the company’s letter head stating name, passport number, designation, the purpose of travel and the mobile number from the applicant
- Original invitation issued by a Ministry, other central executive authorities, state institutions, enterprises or organizations (For Business Visa)
- Original hotel confirmation voucher along with the payment confirmation receipt
- Registration certificate copy of the hotel / tour operator in Ukraine
- Original overseas travel insurance with the sign and stamp pf the insurance company
- Confirmed return air ticket
- Original latest personal bank statement dually signed and stamped by the bank of last 6 months
- Personal Income tax return of the applicant for the last 3 years
- Relationship proof with the company (i.e. MOA if Director, Partnership deed if Partner, Proprietorship certificate if Proprietor and salary slips of last 3 months in employed)
